Chronic Disease Self-Management Program Series

As a member of the Allegany County Community Partnership on Aging, Ardent Solutions wishes to invite you, colleagues, family and friends who are living with a chronic illness and/or those who are caring for someone with a chronic illness to participate in the upcoming Chronic Disease Self-Management Program; scheduled in Wellsville, New York beginning September 29, 2016. The Chronic Disease Self-Management Program is a transformational six-session, health education program that helps participants become more confident about managing their own chronic illness; such as heart disease, arthritis, diabetes, mental illness, asthma, or any other long-term health condition. Participants learn how to better manage their condition while going about their active lives. They also learn skills to help with making decisions and solving problems that relate to their disease.
Developed by the Patient Education Research Center at the Stanford University School of Medicine, the course is taught by two leaders, both are specially trained volunteers who themselves have a chronic condition or care for a loved-one with a chronic illness.  Each workshop will be held over the course of six weeks, meeting once a week.
